The 'Hole' in Holistic Nursing: Contradictions in the Application of Holistic Nursing in Home Care Nursing.

Objective:
The concept of holistic nursing has a strong position in nursing education and practice, and contributes as a transcendent value to nurses’ ideology. Furthermore, there is a common understanding that a holistic approach encompasses a multi-dimensional perspective including physical, social and spiritual needs to patients. The aim of this paper is to explore how holistic nursing is utilized and applied in home care nursing.
Method:
Findings are based on an ethnographic study of home care nursing in two municipalities in Norway, including participant observation and focus-group interviews of eleven registered nurses (RN). Data are analysed with a content analysis approach.
Result:
All RN’s shared a multi-dimensional understanding of ‘holism’ while assessing patients’ need for health care services. Nevertheless, among RN’s radically different comprehensions of whether a holistic approach is suitable while accomplishing health care services to patients was revealed. Differences became foremost evident in a dissimilar understanding of home care nurses responsibility to indulge patients’ social needs. In addition, differences on how to apply the concept of ‘holism’ resulted in dissimilar health care services across municipalities and gave basis for conflict between RN’s within the same municipality.
Conclusion:
There is a common understanding of holistic nursing as multi-dimensional perspective among RN’s. However, there are differences on whether holistic nursing should result in health care services that are ‘holistic’, ie accomplish all patients’ needs. These differences should be explored further since they gave basis for dissimilar health care services across municipalities and conflict among colleagues in home care nursing.

In result of reforms, inspired by concepts related to New Public Management (Rasmussen, 2004; Vabø, 2005), public enterprises in Norway were reorganized in order to enhance cost-control and efficiency in their organisations. Furthermore, these reforms were accompanied by a reinforcement of management control over the labour process in public enterprises. Hence, a major concept of New Public management was the introduction of a ‘new man-agement’ in public enterprises. However, the introduction of ‘new management’ in the public sector did not entail the recruiting of a new type of managers to public enterprises. It rather resulted in ‘old’ managers having to respond to and to manage their enterprises in accordance with the ‘new’ requests.
Many of these ‘old’ managers were nurse managers, since these reforms encompassed more than thirty per cent of all public health care enterprises in Norway. And, since employees in these enterprises provided services which cost a third of the public budget in a Norwegian municipality, new management was a mean to regain control of constantly increasing health care budgets.
The aim of this paper is to analyse and discuss whether the reorganisation of public health care enterprises resulted in any ‘new’ conditions for nurse mangers or not, and to discuss different outcomes in nurse managers’ responses to organisational change.
The analysis of nurse managers’ conditions for management and responses to organisational change builds on data from case studies of four Norwegian nurse managers and their management assistants – utilising focus groups and individual interviews to provide data about their work and their understanding of managing public health care enterprises.
We found that all nurse managers reported ‘new’ working conditions in result of organisational change, whereas an increased responsibility as manager was pointed out as most important. Hence, all nurse managers had appointed employees to become manager assistants in order to cope with this responsibility, through delegation of managerial tasks. Further, all nurse managers reported that organisational change, in contrast to the former organisational structures, resulted in an increased opportunity to restructure and control employees’ work within their enterprises. However, not all managers used this opportunity, which can be regarded as a result of different outcomes in their interpretation of their role as managers, and thereby in their way of managing the public enterprise.
Two types of role interpretation were identified, each emphasising different aspects of being a nurse manager. First, nurse managers shared an understanding of managing public enter-prises as ‘business managers’, emphasising cost and control of employees, a perception that coincide with NPM‘s management concept. Second, at the same time there is a contradictive aspect in that the nurse managers regarded themselves as organisers of professional ser-vices, following professional standards and promoting clients’ needs. The latter view builds on their role as registered nurses and refers to professional norms opposing the NPM concept.
In our concluding discussion we provide a theoretical framework of how to explain the exist-ence of contradicting aspects within nurse management, and the mechanisms that result in different outcomes of these aspects.

In order to break a rule you follow another rule ? there is no such thing as rule-free rule-breaking. In research on employees? opposition to employers, Ackroyd and Thompson?s (1999) notion of organizational misbehaviour (OMB) ? ?self-conscious rule-breaking? ? has had an important impact in several ways. Most significant is perhaps their insistence that resistance and OMB is in fact a common phenomenon at workplaces; all is not quiet on the workplace front (Thompson & Ackroyd 1995) pace what several researchers had claimed. But the importance of other rules than those emanating from the organization itself has been insufficiently analyzed. The aim of this paper is therefore to describe mechanisms that legitimize organizational misbehavior, i.e. rules guiding the breaking of organizational rules. The identification and analysis of work relations and rules developed within these relations give us a better understanding of organizational behaviour and misbehaviour at the workplace. Deliberately breaking a rule involves that the rule-breaker draws on a rule that he or she thinks overrides the first rule. That is why we simply ask: What rules are active when employees consciously break employer rules? Further, we analyse the social structures ? work relations ? which harbour these mechanisms. The analysis of rules and work relations builds on data from a number of case studies of Norwegian health care workers in home-help service ? nurses, assistant nurses and home aid workers ? utilising participant observation, focus groups and individual interviews to provide data about their work and OMB. Although employees carried out the same types of OMB, the rationale for doing so diverged among them, since different occupations called upon dissimilar types of rules to legitimize their misbehaviour. Three types of rules legitimized the overriding of organisational rules, all based on distinctive work relations among employees, including employees? work relations to clients. Although these work relations established rules that legitimized OMB, all work relations depended on and were related to the formal work relationship between employers and employees. First, there were situational rules based on informal work relations with clients ? situational work relations. Although these rules were established across all occupations in the study, situated rules were most active in long-lasting work relations between employees and clients. Second, there were collective rules, developed in consequence of employees? social position at the workplace, bringing employees together in work teams and thereby establishing collective work relations. Collective rules included rules that modified organizational rules on how to provide service to clients, and rules that legitimized the breaking of administrative rules. Finally, professional rules, as a result of professionals? socialisation through formal education and work relations among professionals at the workplace, provided the last distinct type of rules to legitimize organisational misbehaviour. Altogether, situational, collective and professional rules provide distinct types of legitimacy to OMB and are active at the workplace, although there are differences between employees concerning what type of rules they call upon when breaking organizational rules.

The paper, based on an empirical study, presents a theoretical framework of a normative structure in professional work. This normative structure consists of an administrative, a collective and a professional norm system, that are concepts encompassing formal, informal and professional norms. These norm systems are distinctive in their characteristics, but still interrelated, and hierarchically assessed by professionals at work. Although each norm system has an influence on professionals in work-related processes, the relative outcome of each norm system depends on the interrelationship of all systems.

Presentasjonen er knyttet til en studie av bestiller-/utførermodellen (BUM) i den hjemmebaserte tjenesten (nødvendig helsehjelp/praktisk bistand) i to av kommuner. Fokuset for studien er de ansatte som yter nødvendig helsehjelp og praktisk bistand i en virksomhet organisert etter BUM og deres arbeidsvilkår som følge av bestiller-/utførermodellen. De preliminære funn viser at ulike operasjonalisering medfører ulike endringer i arbeidsvilkårene og utfordrer ulike forståelser av omsorgsarbeidet.

Bakgrunn og formål. De helsefremmende og helseforebyggende effektene ved fysisk aktivitet er dokumentert i litteraturen. Videre er det dokumentert at over halvparten av den voksne befolkningen er fysisk inaktive. Formålet med denne studien var å kartlegge det fysiske aktivitetsnivået blant eldre og å finne frem til faktorer som påvirker variasjonene i de eldres aktivitetsnivå. Metode. I løpet av en observasjonsuke høsten 2005 svarte et tilfeldig utvalg på 111 hjemmeboende eldre fra en bykommune i Østfold på et spørreskjema. Spørreskjemaet ble distribuert blant kommunens fastlegekontorer og innholdt spørsmål om fysisk aktivitet og ulike påstander som begrunnet aktivitetsnivået. Resultater. Studien viser at de fleste eldre har et aktivitetsnivå som ikke samsvarer med en anbefaling om en halv time med daglig fysisk aktivitet for å kunne høste en helsegevinst. De viktigste faktorene som hindret de eldre i det å være fysisk aktive er angsten for å skade seg og angsten for å gå ut alene.
